Where to buy a good muzzle and how to stop nuzzling

Well, I'm not sure if this is appropriate to this site, but I am in need of a good muzzle for my German Shepherd Dog. He is almost two years old and has anxiety issues and fear aggression. His obedience trainer/behaviorist and his vet suggested a muzzle, mostly for visits to the vet. He barks and lunges at people just trying to pass by on the street and at the vet he is a holy terror, so for everyone's safety I think it is a good idea to buy him one. He will not hesitate to go after a stranger if they get too close. Believe me I have tried everything else. We've tried meds for his aggression and anxiety, but it just made him lethargic, we've tried the relaxation protocol, we're currently attempting behavior modification, and we've tried showing him the vet isn't such a bad place by just taking him there to get treats. But he runs in the vet every time, agitated and guns blazing ready to bark at whomever he sees. We've already started training him to get used to a muzzle using a few of the one's his obedience trainer has let us borrow, but I would really like to get him a custom made muzzle. It has to be a strong enough muzzle that he cannot bite anyone while he is wearing it, something like a cage muzzle, but he also needs to be able to take treats with the muzzle on as I clicker train him. So if anyone knows a good place online where I can buy him a custom made muzzle, it would be much appreciated!

Also, is there anyway to get my dog to stop nuzzling? I'm kind of short, and my dog is quite large, so when he nuzzles me it's right at that divot under my hip which kind of hurts. He always nuzzles in protest too, when he doesn't want to do something. Like he nuzzles my hip when he doesn't want to leave the park, or when he doesn't want to wear his head halter, or when he doesn't want to go inside after playing outside for awhile. It has grown to be quite an annoying habit of his and it drives me nuts.
